ALL SPRING INCOMING STUDENTS NEED TO COMPLETE THE FOLLOWING TWO MEDICAL REQUIREMENTS BY FEBRUARY 1, 2024:

1. TUBERCULOSIS SCREENING: Complete your Tuberculosis (TB) Screening Form on Med+Proctor. This screening will occur automatically upon registering with Med+Proctor. Follow-up skin testing (tuberculin testing) or blood testing (example: Quantiferon Gold) and/or a chest x-ray may be needed following this screening. 

2. IMMUNIZATIONS: Submit a copy of your immunization record to Med+Proctor, a FERPA & HIPAA compliant third-party immunization verification service, so that your required immunizations can be verified.  You will need to register for a new account using your SMC email.

·  PLEASE NOTE:  No payment is required to complete this process. Students only need to provide their SMC email, student ID and name to create their account. After registering your information and completing your free online profile, you will be given the option to pay an optional one-time fee of $10 for Med+Proctor membership benefits, but this is NOT required to submit your records.  Required immunizations:

  • Meningococcal (ACWY): Required for all students 21 years of age or younger. Must include one dose on or after age 16.
  • MMR (Measles, Mumps, and Rubella): two doses or positive MMR titer documenting immunity.
  • Tdap (Tetanus, Diphtheria, and Pertussis): one dose within the last 10 years.

Please see your healthcare provider if you are missing these vaccines, or they can be scheduled through many pharmacies like CVS or Walgreens.

Clinician-signed medical exemption forms will be accepted if applicable. These should be submitted to Med+Proctor. Please utilize their help desk or live chat if you have questions.

Additional highly recommended immunizations (not required):

  • Meningococcal B: A series of 2 or 3 injections depending on vaccine used. http://www.cdc.gov/meningococcal/vaccine-info.html
  • Hepatitis B: 3 doses
  • Hepatitis A: two doses
  • HPV: three doses
  • Polio: if did not complete series as a child
  • Varicella (Chicken Pox): two doses of vaccine, history of Chicken Pox, or positive Varicella titer documenting immunity.
  • Annual influenza vaccine
  • COVID-19: at least one bivalent vaccine dose (August 2022 or later)
